Diet Follow-Up

Monthly diet fee for those currently established in the past 2 years

B-12 Injections

1 single B-12 injection

Lipo-B12 Injections

1 Lipotropic injection. Includes: methionine, inositol, choline, B6 and B12. This injection is designed to help your body release stored fat and speed up your metabolism so you can burn fat faster than usual.

Physicals & Employment Services

DOT Physical (Med Card)

$80

A physical exam that is required for individuals who wish to operate & drive a commercial vehicle weighing over 10,000 lbs.

Pre-Employment Physical

A medical examination of a job-candidate's overall health, medical history & physical abilities to determine if they are fit for a job

Sports Physical

$25

Required for students to participate in school sports

Federal, Forensic & 12 panel (Rapid) Drug Testing

We can perform urine testing for: Pre-employment, random, post accidents, etc.
